Cavernous Hemangioma
What is the most likely diagnosis? → Cavernous Hemangioma
Cavernous Hemangiomas
- Composed of large, dilated vascular channels
- More infiltrative than capillary type; frequently involve deep structures; do not spontaneously regress
- Difficult to distinguish from malignant counterparts on imaging
- Histology: sharply delineated but unencapsulated ; large cavernous blood-filled vascular spaces separated by connective tissue stroma
- Intravascular thrombosis with dystrophic calcification common

Cavernous Hemangioma
- Large, dilated, blood-filled vascular channels separated by connective tissue stroma
- Infiltrative and poorly circumscribed — does NOT regress
- Contains intravascular thrombi, dystrophic calcification, haemosiderin
- Can be difficult to distinguish from malignancy on imaging
- Association: Von Hippel-Lindau disease — cerebellar, brain stem, retinal, pancreatic, and hepatic cavernous hemangiomas

What our notes say
- What is the most likely diagnosis? → Cavernous Hemangioma (Liver Tumors: Benign and Malignant Overview)
- Association: Von Hippel-Lindau disease — cerebellar, brain stem, retinal, pancreatic, and hepatic cavernous hemangiomas (VASCULAR PATHOLOGY — Must-Know Comprehensive Notes)
- | Cavernous hemangioma | Infiltrative; no regression; Von Hippel-Lindau; never biopsy percutaneously in liver | (VASCULAR PATHOLOGY — Must-Know Comprehensive Notes)
- Benign Tumours — Capillary Hemangioma · Cavernous Hemangioma · Pyogenic Granuloma · Lymphangioma · Glomus Tumour (VASCULAR PATHOLOGY — Must-Know Notes)
- | Cavernous hemangioma | Deep, infiltrative, no regression; von Hippel-Lindau association; brain = problematic | (VASCULAR PATHOLOGY — Must-Know Notes)
- Most Common Benign: Cavernous Hemangioma ⭐ - Most common benign liver tumor - Usually incidental finding - Do NOT biopsy (risk of massive hemorrhage) ⭐ (Liver Histology & Physiology)
