Hypertrophic Cardiomyopathy
Also written as HCM.
Hypertrophic cardiomyopathy murmurs get louder with Valsalva and are less likely to radiate to the carotids.
Hypertrophic Cardiomyopathy (HCM)
- Massive hypertrophy of left ventricle especially asymmetric septal hypertrophy
- Small or normal cavity size; diastolic dysfunction (stiff ventricle, impaired filling)
- Most common cause of sudden cardiac death in young athletes
- Cause: Autosomal dominant mutations in sarcomere proteins (β-myosin heavy chain, troponin T, troponin I, myosin binding protein C)
- Asymmetric septal hypertrophy

Hypertrophic Cardiomyopathy (HCM/HOCM)
- Autosomal dominant — beta-myosin heavy chain mutation (MYH7)
- Asymmetric septal hypertrophy + myofiber disarray
- Dynamic LVOT obstruction — worsens with decreased preload (standing, Valsalva)
- cause of sudden cardiac death in young athletes
- Auscultation: harsh systolic murmur at left sternal border
