Lynch Syndrome
Also written as HNPCC.
Risk factors (unopposed oestrogen): age 55 to 65, obesity, nulliparity, early menarche and late menopause, anovulation and PCOS, oestrogen-only HRT, tamoxifen, oestrogen-secreting ovarian tumours, diabetes, hypertension, and a family history (Lynch syndrome).
What our notes say
- Risk factors (unopposed oestrogen): age 55 to 65, obesity, nulliparity, early menarche and late menopause, anovulation and PCOS, oestrogen-only HRT, tamoxifen, oestrogen-secreting ovarian tumours, diabetes, hypertension, and a family history (Lynch syndrome). (Junior Clerkship in Reproductive Health (MBOG 4200): end of year examination, 4 )
- Risk factors: familial adenomatous polyposis, hereditary non-polyposis colorectal cancer (Lynch syndrome), inflammatory bowel disease, low-fibre diet, adenomatous polyps (Pathology Practical Revision Guide)
- *Pathogenesis:* Adenoma-carcinoma sequence (APC, KRAS, p53 mutations); APC mutation typically initiating; further mutations drive invasion; alternative pathway via microsatellite instability (Lynch syndrome) (Medical MCQs: Chemical Pathology & Haematology Exam Review)
- Family history, BRCA1/2 mutations, Lynch syndrome (HNPCC) (Ovarian & Colorectal Cancer: Risks, Pathogenesis, Staging)
- Underlies Lynch syndrome (HNPCC) — autosomal dominant, right-sided predominance, earlier onset, less step-wise polyp progression (Ovarian & Colorectal Cancer: Risks, Pathogenesis, Staging)
- Microsatellite instability pathway: MLH1/MSH2 defects → Lynch syndrome, right-sided, less polyp-dependent (Dr. Irungu Notes)
