RHEUMATIC HEART DISEASE
- Cause: Group A Strep pharyngitis → autoimmune cross-reaction (type II/III hypersensitivity)
- Anti-streptolysin O (ASO) antibodies cross-react with cardiac tissue
- Aschoff bodies — pathognomonic granulomas with Anitschkow cells (caterpillar cells)
- Jones Criteria (major): Carditis, Polyarthritis, Chorea, Erythema marginatum, Subcutaneous nodules
- Most common valve affected: Mitral Aortic Tricuspid

Practice questions on Rheumatic Heart Disease
A 24-year-old woman with rheumatic heart disease receives a porcine bioprosthesis. After 10 years it needs replacement. Most likely pathologic finding?
- Dehiscence
- Endocarditis
- Strut failure
- Calcification
Answer: D. Bioprosthetic valves avoid anticoagulation but undergo progressive calcification over 5–10 years, causing stenosis and requiring replacement. ---
Which of the listed below is LEAST commonly affected in Rheumatic heart disease?
- Mitral Valve
- Tricuspid Valve
- Mitral and Aortic valve
- Aortic Valve
- Pulmonary Valve ✅ Answer: E
Answer: A. The pulmonary valve is the least commonly affected valve in rheumatic heart disease, which predominantly affects the mitral and aortic valves.
